Why additional aligners are common and what should be reassessed before a refinement starts.

A refinement is a new planning stage

If teeth do not finish exactly where intended, new records can be taken and another aligner sequence designed. This is common enough that patients should ask about it before treatment starts.

First determine why the original sequence fell short

The cause may be biological response, inconsistent wear, attachment loss, insufficient space or the limits of the original design. A refinement should address the cause rather than simply repeat the same assumptions.

Fees and time windows vary

Some treatment packages include additional aligners for a defined period; others may charge after certain limits. Clarify the policy in writing.

Refinement is not the same as perfectionism

The provider and patient should agree on the remaining goals and whether additional movement is clinically worthwhile before extending treatment.

Questions to take to a consultation

  • What is the diagnosis and treatment objective?
  • Who will personally diagnose, plan and supervise treatment?
  • What reasonable alternatives exist?
  • What is included in the total fee, including refinements and retainers?
  • How often are in-person visits expected?
